Vincristine Sulfate Injection, USP (vincristine sulfate) is the salt of an alkaloid obtained from a common flowering herb, the periwinkle plant (Vinca rosea Linn). Originally known as leurocristine, it has also been referred to as LCR and VCR. The molecular formula for Vincristine Sulfate, USP is C46H56N4O10•H2SO4. It has a molecular weight of 923.04.

5% Sodium Bicarbonate Injection, USP (sodium bicarbonate (sodium bicarbonate 5% injection) 5% injection) is a sterile, nonpyrogenic solution of Sodium Bicarbonate (sodium bicarbonate 5% injection) , USP in Water for Injection, USP. It contains no antimicrobial agent. Composition, osmolarity, pH and ionic concentration are shown below.

Vasopressin Injection, USP is a sterile, aqueous solution of synthetic vasopressin (8-L-arginine vasopressin) of the posterior pituitary gland for intramuscular or subcutaneous use. It is substantially free from the oxytocic principle and is standardized to contain 20 pressor units/mL.

ProHIBiT®, Haemophilus b Conjugate Vaccine (Diphtheria Toxoid-Conjugate), for intramuscular use, is a sterile solution, prepared from the purified capsular polysaccharide, a polymer of ribose, ribitol and phosphate (PRP) of the Eagen Haemophilus influenzae type b strain covalently bound to diphtheria toxoid (D) and dissolved in sodium phosphate buffered isotonic sodium chloride solution. The polysaccharide-protein conjugate molecule is referred to as PRP-D. Thimerosal (mercury derivative) 1:10,000 is added as a preservative. The vaccine is a clear, colorless solution. Each single dose of 0.5 mL is formulated to contain 25 ?µg of purified capsular polysaccharide and 18 ?µg of diphtheria toxoid protein.

FLUORESCITE® (fluorescein injection, USP) 10% contains fluorescein sodium (equivalent to fluorescein 10% w/v). It is a sterile solution for use intravenously as a diagnostic aid. Its chemical name is spiro[isobenzofuran-1(3H),9'-[9H]xanthene]-3-one, 3'6'-dihydroxy,disodium salt. The active ingredient is represented by the chemical structure:

LIPITOR (atorvastatin calcium) is a synthetic lipid-lowering agent. Atorvastatin is an inhibitor of 3-hydroxy-3-methylglutaryl-coenzyme A (HMG-CoA) reductase. This enzyme catalyzes the conversion of HMG-CoA to mevalonate, an early and rate-limiting step in cholesterol biosynthesis.

DTlC-Dome Sterile (dacarbazine) is a colorless to an ivory colored solid which is light sensitive. Each vial contains 100 mg of dacarbazine, or 200 mg of dacarbazine (the active ingredient), anhydrous citric acid and mannitol. DTlC-Dome is reconstituted and administered intravenously (pH 3–4). DTlC-Dome is an anticancer agent. Chemically, DTlC-Dome is 5-(3, 3-dimethyl-l-triazeno)-imidazole-4-carboxamide (DTlC) with the following structural formula:

Furoxone (furazolidone) is one of the synthetic antimicrobial nitrofurans. It is a stable, yellow, crystalline compound 3-(5-nitrofurfurylideneamino)-2-oxazolidinone .

Xylocaine (lidocaine HCl) Injections are sterile, nonpyrogenic, aqueous solutions that contain a local anesthetic agent with or without epinephrine and are administered parenterally by injection.

HYCODAN (hydrocodone bitartrate and homatropine methylbromide) contains hydrocodone (dihydrocodeinone) bitartrate, a semisynthetic centrally-acting opioid antitussive. Homatropine methylbromide is included in a subtherapeutic amount to discourage deliberate overdosage.

Clindamycin hydrochloride is the hydrated hydrochloride salt of clindamycin. Clindamycin is a semisynthetic antibiotic produced by a 7(S)-chloro-substitution of the 7(R)-hydroxyl group of the parent compound lincomycin.

Positive ANA Titer: The prolonged administration of procainamide often leads in the development of a positive anti-nuclear antibody (ANA) test, with or without symptoms of a lupus erythematosus-like syndrome. If a positive ANA titer develops, the benefits versus risks of continued procainamide therapy should be assessed.

RAPAFLO is the brand name for silodosin, a selective antagonist of alpha-1 adrenoreceptors. The chemical name of silodosin is 1-(3-Hydroxypropyl)-5-[(2R)-2-({2-[2-(2,2,2-trifluoroethoxy)phenoxy]ethyl}amino)propyl]-2,3-dihydro-1H-indole-7-carboxamide and the molecular formula is C25H32F3N3O4 with a molecular weight of 495.53. Cyclessa® (desogestrel ethinyl estradiol tablets) (desogestrel/ethinyl estradiol) Tablets are a triphasic oral contraceptive containing two active components, desogestrel and ethinyl estradiol. Each 28-day treatment cycle pack consists of three active dosing phases: 7 light yellow tablets containing 0.100 mg desogestrel (13-ethyl-11-methylene-18,19-dinor-17? -pregn-4-en-20-yn-17-ol) and 0.025 mg ethinyl estradiol (19-nor-17? -pregna-1,3,5(10)-trien-20-yne-3, 17-diol); 7 orange tablets containing 0.125 mg desogestrel and 0.025 mg ethinyl estradiol, and 7 red tablets containing 0.150 mg desogestrel and 0.025 mg ethinyl estradiol. Inactive ingredients include vitamin E, pregelatinized starch, stearic acid, lactose monohydrate, hydroxypropyl methylcellulose, polyethylene glycol, titanium dioxide, talc, yellow ferric oxide (in light yellow and orange tablets), and red ferric oxide (in orange and red tablets). Cyclessa® (desogestrel ethinyl estradiol tablets) also contains 7 green tablets with the following inert ingredients: lactose monohydrate, corn starch, magnesium stearate, hydroxypropyl methylcellulose, polyethylene glycol, titanium dioxide, FD&C Blue No. 2 aluminum lake, yellow ferric oxide, and talc. CIPRO XR (ciprofloxacin* extended-release tablets) contains ciprofloxacin, a synthetic broad-spectrum antimicrobial agent for oral administration. CIPRO XR tablets are coated, bilayer tablets consisting of an immediate-release layer and an erosion-matrix type controlled-release layer. The tablets contain a combination of two types of ciprofloxacin drug substance, ciprofloxacin hydrochloride and ciprofloxacin betaine (base). Ciprofloxacin hydrochloride is 1 -cyclopropyl-6-fluoro-1,4-dihydro-4-oxo-7- (1 -piperazinyl)-3-quinolinecarboxylic acid hydrochloride. It is provided as a mixture of the monohydrate and the sesquihydrate. The empirical formula of the monohydrate is C17H18FN3O3 • HCl • H2O and its molecular weight is 385.8. The empirical formula of the sesquihydrate is C17H18FN3O3 • HCl • 1.5 H2O and its molecular weight is 394.8. The drug substance is a faintly yellowish to light yellow crystalline substance. Adacel® , Tetanus Toxoid, Reduced Diphtheria Toxoid and Acellular Pertussis Vaccine Adsorbed (Tdap), is a sterile liquid suspension of tetanus and diphtheria toxoids and acellular pertussis components adsorbed onto aluminum phosphate, for intramuscular administration. After shaking, the vaccine is a white, homogenous, cloudy suspension. Neither the vial nor the syringe for this product contain natural latex rubber.

ASIMIATM (paroxetine mesylate) is an orally administered psychotropic drug with a chemical structure related to paroxetine hydrochloride (Paxil®). It is the mesylate salt of a phenylpiperidine compound identified chemically as (-)-trans -4R- (4' - fluorophenyl) - 3S - [(3', 4-methylenedioxyphenoxy) methyl] piperidine mesylate and has the empirical formula of C 19H20FNO3 CH3SO3H. The molecular weight is 425.5 (329.4 as free base). The structural formula is:

Prochlorperazine Maleate is classified as an antiemetic and antipsychotic tranquilizer. Prochlorperazine is a phenothiazine derivative, present in the tablets as the maleate. Its chemical name is 2-chloro-10- [3-(4-methyl-1-piperazinyl)propyl]-10 H-phenothiazine (Z)-2-butenedioate (1:2). Empirical formulas (and molecular weight) are: prochlorperazine maleate-C20H24CIN3S•2C4H4O4 (606.10) and prochlorperazine base- C20H24CIN3S (373.95).

CORTEF Tablets contain hydrocortisone which is a glucocorticoid. Glucocorticoids are adrenocortical steroids, both naturally occurring and synthetic, which are readily absorbed from the gastrointestinal tract. Hydrocortisone USP is white to practically white, odorless, crystalline powder with a melting point of about 215° C. It is very slightly soluble in water and in ether; sparingly soluble in acetone and in alcohol; slightly soluble in chloroform.

TARCEVA (erlotinib), a kinase inhibitor, is a quinazolinamine with the chemical name N-(3-ethynylphenyl)-6,7-bis(2-methoxyethoxy)-4-quinazolinamine. TARCEVA contains erlotinib as the hydrochloride salt that has the following structural formula:

Dalmane (flurazepam) is available as capsules containing 15 mg or 30 mg flurazepam hydrochloride. Daytrana™ (methylphenidate transdermal system) is an adhesive-based matrix transdermal system (patch) that is applied to intact skin. The chemical name for methylphenidate is ?phenyl-2-piperidineacetic acid methyl ester. It is a white to off-white powder and is soluble in alcohol, ethyl acetate, and ether. Focalin® (dexmethylphenidate hydrochloride) is the d-threo-enantiomer of racemic methylphenidate hydrochloride, which is a 50/50 mixture of the d-threo and l-threo-enantiomers. Focalin (dexmethylphenidate hydrochloride) is a central nervous system (CNS) stimulant, available in three tablet strengths. Each tablet contains dexmethylphenidate hydrochloride 2.5, 5, or 10 mg for oral administration. Dexmethylphenidate hydrochloride is methyl ?-phenyl-2-piperidineacetate hydrochloride, (R,R')-(+)-. MEVACOR (Lovastatin) is a cholesterol lowering agent isolated from a strain of Aspergillus terreus. After oral ingestion, lovastatin, which is an inactive lactone, is hydrolyzed to the corresponding ?hydroxyacid form. This is a principal metabolite and an inhibitor of 3-hydroxy-3-methylglutarylcoenzyme A (HMG-CoA) reductase. This enzyme catalyzes the conversion of HMG-CoA to mevalonate, which is an early and rate limiting step in the biosynthesis of cholesterol.

ALFENTA (alfenta (alfenta (alfenta (alfentanil) nil) nil) nil hydrochloride) Injection is an opioid analgesic chemically designated as N-[1-[2-(4-ethyl-4,5-dihydro-5-oxo-1H-tetrazol-1-yl)ethyl]-4- (methoxymethyl)-4-piperidinyl]-N-phenylpropanamide monohydrochloride (1:1) with a molecular weight of 452.98 and an n-octanol:water partition coefficient of 128:1 at pH 7.4.
